Enable intrange linter to enforce modern Go range syntax over
traditional for loops, by converting:
for i := 0; i < n; i++
to:
for i := range n
Adding type conversions where needed for compatibility
with existing uint64 parameters.

Wrap d in a mutex to prevent data race. This makes is slower, but this
is a debugging aid anyway. It's not used normally.

Allow plugins to dump messages in text pcap to the log. The forward
plugin does this when a reply does not much the query.
If the debug plugin isn't loaded Hexdump and Hexdumpf are noop.
